passport, at least 6 months valid on application

1 application form, fully completed

1 photograph

itinerary and hotelconfirmation (with hotel address!), hotel details must be mentioned on appication form

If the applicant mentions to be unemployed or housewife a copy of a recent bank statement (min. € 500) is required
